OKDHS accepting replacement SNAP requests after November storms
OKLAHOMA CITY (KFOR) — Oklahoma Human Services is now accepting requests for replacement SNAP benefits after several counties in the state were impacted by severe storms in November.
SNAP benefit recipients in Cleveland, Garvin, Lincoln, McClain, Oklahoma and Stephens counties who lost food due to tornadoes or power outages may qualify to get some benefits back.

Those effected can apply for benefit replacement using the " Request for Destroyed Food Replacement " form within 10 days of their food loss. If you need assistance filling out the form, contact OKDHS at 405-522-5050.
For those who may have lost their SNAP benefits card during the storms, here are three ways you can get a replacement card:

By contacting Conduent at 1-888-328-6551 or Oklahoma Human Services at (405) 522-5050; or
